Shirley T. Addis, 88, formerly of Hennepin, died Nov. 27, 2015, in Heartland Healthcare, Henry.

Cremation rites have been accorded. There will be a private service at a later date.

Dysart-Cofoid Funeral Chapel will assist the family with arrangements.

Shirley was born Sept. 16, 1927, in Canonsburg, Pa., to James and Ada (Jones) Champ [later Ada Samuels]. She married William Addis on Nov. 8, 1946, in Canonsburg.

After graduating high school, she attended School of Design in Pittsburgh. She was a member of the Order of Eastern Star of the Grand Chapter in Pennsylvania and was a volunteer for Meals On Wheels.

Shirley is survived by two daughters, Cheryl (Mike) Pezzelle of Phoenix, Ariz., and Elizabeth (John) Ciucci of Hennepin; one granddaughter-in-law, Mandy Ciucci; four grandchildren, Mike (Megan) Pezzelle, Mark (Stephanie) Pezzelle, Matt (Renee) Pezzelle and Keith (Stephanie) Ciucci and 12 great-grandchildren.

She was preceded in death by her parents; her husband, William, in 2012; her grandson, Eric Ciucci in 2014; and one brother and three sisters.
