MOSES MOUNTAINVILLE ADDINGTON was born 17 April 1879 in Carrolton Township, Boone County, Arkansas, USA, the son of Moses Wiley Addington and his wife, Mary Katherine "Katie" Sipe. Like his father, he often went by the nickname of "Mode" or "Modie."

There are some conflicting reports about Moses' birth location. In the 1880 census the family was in Carrolton Township, Boone County, Arkansas, USA. (Carrolton is not a choice that can be selected in the drop-down FindAGrave menus). By the 1900 census, when he was in Idaho, Moses gave his birth location as Missouri, although in all other censuses he gives Arkansas as the birth location. In his WWII draft registration card (filled out in 1942), Moses said he was born in Harrison, Arkansas. Harrison is the county seat of Boone County, Arkansas, so Modie may just have given the nearest large town as his birth location — or it's possible that he was actually born in Harrison for some reason, even though his father was a laborer in nearby Carrolton Township. Today, of course, Carrolton Township is considered part of the Harrison metro area. Finally, in his Social Security death claim (filed 16 years after his death), his birth location was given — most likely erroneously — as Little Rock, Arkansas.
